Success Story: Andrew S.

Mr. Andrew S. admitted to Woodridge Nursing of Grapevine following a hospital stay where he suffered from a sudden onset of weakness due to pneumonia and sepsis. He was so debilitated upon his arrival, he required maximum assistance from staff to help him with dressing, bathing, transferring and walking. He was determined to return home to his wife and regain his independence, so the therapy team collaborated and tailored his plan to fit his needs.

Andrew worked five days per week with the rehab team, focusing on strength, balance, and activities of daily living. He was able to improve his ability to transfer and walk to a stand-by assist level as well as fulfilled his daily self care goals of reaching a level of modified independence. After a short seven week stay with us, he was able to meet most of his long term goals and discharge home, where he continued his therapy with Home Health services. We thank you Andrew for allowing us to assist you in your rehab needs and wish you all the best as you continue to recover. (Source Model 10/ 0 is Dependent and 10 is Independent)
